There have been recent changes in the way special recognition license plates are handled with the BMV. These changes reflect "renewals only", not new plates or transfer plates. We are now able to submit the authorization form directly to the BMV. Please read below to see which category applies to you.
FOP Special Recognition License Plates are for active and retired F.O.P. members only.
If you are renewing a (FOP) license plate only & wish to have sent to the BMV electronically: DO NOT APPLY FOR ELECTRONIC PROCESS IF YOUR VEHICLE IS A TRANSFER OR A NEW PLATE, THIS IS FOR RENEWALS ONLY WITH NO CHANGES FROM THE PREVIOUS YEAR.
You no longer need to pick up a form with a seal, if you choose not to. For renewals only (this does not include new plates or transfer plates), Once we have verified your membership and submitted electronically for you, it is then your responsibility to go to www.in.gov and forward. Or you can go into your local branch and pick up your renewal sticker temporary. Please allow a four hour turn-around time to process. If you submit your information to renew on our web-site after 4:30 PM, please allow us until 12:30 PM the following business day to submit to the BMV. We do need the following information in order to process your request. Without this information, your license plate renewal cannot be processed.

If you are applying for a new, renewal or transfer (FOP) plate and wish to receive a hard copy, with seal, by mail
If you are applying for a new, renewal or transfer plate by mail. The plates are $30.00 per plate. Payment method is check, money order or credit card. The new authorization forms will continue to be sent to you by mail to your home address on your application.
The application is for the Secretary's Office to verify your membership in the Fraternal Order of Police of Indiana. After verification an authorization form will be mailed to you. The authorization name must be identical to the name on the registration.
The authorization form must accompany your vehicle registration form when you purchase your plates. You can use our secure server or you can print out the form below.
Apply for new, renewal or transfer plate(s) online using our secure payment form. (HARD COPY WILL BE SENT BY MAIL)
Please be aware that if you are receiving your copy electronically please allow a four hour turn around time M-F and before 4:30 p.m. (excluding holiday's). If you are receiving a hard copy by mail, your paperwork is submitted that day and mailed that day "as long as membership is current". Post Office usually takes 1-3 days for delivery. If you paid for your plate(s) by credit card, know that we do not hold up the process of your plate, but we will not run your credit card until the end of that business week. Example: If a plate order is requested on a Monday and during our buisness hours it will be processed that day either by mail or electronically to the BMV. The credit card provided for the plate(s) will be processed at the end of that week, typically a Friday.
