All plate authorizations ordered in 2019 will be mailed to you by hard copy. 2019 is the start of the new plate cycle and everyone will be receiving a new plate for their vehicles if you are at the end of that seven year cycle. The electronic process was for renewals only and since that does not apply for the 2019 year we have removed from our site. The electronic feature that we have been using for the last several years will be available to you again in the 2020 plate cycle.
You can still apply on-line by credit card, download the application and mail in, or call the office and apply for your form.
If you are applying for a new, renewal or transfer plate by mail. The plates are $30.00 per plate. Payment method is check, money order or credit card. The new authorization forms will continue to be sent to you by mail to your home address on your application.
The application is for the Secretary's Office to verify your membership in the Fraternal Order of Police of Indiana. After verification an authorization form will be mailed to you. The authorization name must be identical to the name on the registration.
The authorization form must accompany your vehicle registration form when you purchase your plates. You can use our secure server or you can print out the form below.
If you are receiving a hard copy by mail, if your paperwork is submitted that day and mailed that day "as long as membership is current", it's not after hours and is not the weekend and/or a holiday. Post Office usually takes 1-3 days for delivery. If you paid for your plate(s) by Credit Card, know that we do not hold up the process of your plate, but we will not run your credit card until Friday of that week.
